    Types of hypoacusis

    Types of hypoacusis

    In medical terms, hearing loss is called hypoacusis, and there are different types and different symptoms.

    Transmissive hypoacusis
    With damage in the outer ear or in the middle ear, i.e. in those parts of the ear responsible for the mechanical transmission of sound. Subjects affected by this type of hearing loss hear all the low tones, weakened independently to the type of frequency. They have the feeling their ears are plugged and speak softly because they hear them unusually loudly.

    Neurosensory hypoacusis
    Concerns the inner ear which becomes incapable of transforming the sound vibrations into correct nerve impulses. Subjects affected by this type of hypoacusis hear without understanding: they do not recognize the sounds.

    Mixed type hypoacusis
    This occurs when the injury affects both the middle ear and the inner ear at the same time. Obviously, with this type of hearing loss the above two types combine.

    Central hypoacusis
    The center of hearing is located in the brain. Sometimes, the sounds sent by the ear are not correctly interpreted. This is called central deafness.

    Whistling
    these are all those annoying noises and sounds that a person with this problem says he can hear. They can be more or less associated with hypoacusis. It is not a case of weaker hearing, however it does cause a series of difficulties in understanding words and sounds which are covered by these noises.

    Typical symptoms of hypoacusis

    Save for rare cases, hypoacusis is not a sudden process, but comes on slowly and gradually, determining a kind of habituation preventing it from being consciously recognized.

    In the beginning not all sounds disappear, but only a few. The first and most common symptoms of hearing loss are:

    The need to have sentences repeated
    The hypoacusis patient, i.e. the person who hears less, misses words and phrases because he cannot understand them or confuses them, but also because he is hearing the lower frequency noises (electric household appliances, traffic, etc.) louder, which become annoying.

    Difficulty in interpersonal relations
    If several people are speaking together, if a conversation occurs in traffic or in a car, understanding becomes difficult.

    Errors in understanding speech
    The first sounds not to be heard are the high tones. Words including low tones, such as the vowels AEIOU, and the high frequencies contained especially in consonants, like CSFZ. Therefore if certain consonant letters are not understood it is because they are not heard (high tones).

    Discomfort and stress
    Difficulty in understanding words during a conversation makes participation in social life tiring.

    Difficulty in understanding children
    Children voices are shriller and therefore higher pitched.

    Symptoms of Hypoacusis

    The loss or reduction of hearing ability, if it is not the result of an accident or an illness, is usually an easily-recognised process, indicated by a worsening of certain hearing experiences.

    Looking at this in detail, it appears that the most common symptoms among patients who have a gradual hypoacusis are:

    The need for words to be repeated
    The hard-of-hearing miss words and phrases either because they don't understand them or because they confuse them with other words or because they can hear, at the same time, low-frequency sounds (such as electrical household appliances or traffic), which become irritating.

    Difficulty in understanding a conversation in certain circumstances
    If more than one person is speaking at the same time, if the conversation takes place in traffic or inside a car, understanding is more difficult.

    Misunderstandings in conversation
    The first sounds, which are not heard are the 'high tones'. Words contain both low frequency sounds, as in vowels for example, and high frequency sounds, mainly in some consonants. Some words can be misunderstood or not heard especially when spoken by children who have shriller voices with higher tones.

    Causes of Hypoacusis

    The worsening of hearing ability is the subject of many detailed studies; research to date has indicated several factors that contribute to partial or total loss of hearing.

    Age
    Throughout the world, statistics reveal that the process of human ageing is definitely one of the most important causes of some kind of hearing deficit. A high percentage, about 30%, of the population aged over 65 suffer and it appears to be one of the most common complaints among elderly people.

    Noise
    Noise, on the other hand, is the primary cause of professional invalidity. The deafening background noise of daily life is not only irritating, it causes damage. Not only is hearing affected but the whole body suffers with serious consequences such as increased blood pressure, tachycardia, arrhythmia, heartburn, muscular tension, intestinal problems, and so on.

    Heredity
    About one third of hearing difficulties present at birth are caused by hereditary factors. One gene has been indicated as responsible for this: connexion 26. If there are cases of hypoacusis in the family, there is a high probability that this will be transmitted to the children. If two hard-of-hearing people get married, it is advisable to consult an audiologist or a geneticist, a specialist in hereditary illnesses.

    Infections
    Several bacterial and viral illnesses (scarlet fever, rubella, meningitis and others) can damage the ear and cause some degree of hearing loss. Otitis, a common illness among young children, can cause irreversible hypoacusis if not treated in time.

    Otosclerosis
    This is an illness that affects the mobility of the small bones in the middle ear. The vibrations transmitted to the inner ear no longer have the same regularity as those in a healthy ear and so hearing efficiency is reduced.

    Medicines, alcohol, smoking
    It has been shown that some medicines and some substances such as alcohol and nicotine can be harmful to the ear and are defined as ototoxic i.e. toxic for hearing.
